NAPS Fall Preferred Enrollment Commitment Form

THIS FORM MUST BE FILLED OUT FOR EACH CHILD IN YOUR HOME BY MONDAY, AUGUST 10TH AT 5PM. The responses provided in this form will be used for fall reopening decisions in relation to the school scheduling options and transportation. North Adams Public Schools will continue to release additional information to families based on responses to this enrollment commitment form.
